The Fox Chapel Area athletic department is pleased to announce that we will offer the convenience of online registration for all interscholastic sports programs at Dorseyville Middle School and Fox Chapel Area High School, beginning with the 2013-2014 winter sports season. It is accessible through our partner company, FamilyID (www.familyid.com).
FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register your student-athlete and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, you enter your information once for multiple uses, multiple kids and multiple programs.
Please note that sections 5 and 6 of the PIAA physical form must be completed and hand signed by the physician and parent or guardian. The documents and the link to Family ID can be found by accessing our website at www.fcasdathletics.org and follow the links to the Family ID registration (which is located on the right hand side of the page under the black Schedule Star link). Again, sections 5 and 6 of the PIAA physical form must be submitted directly to the athletic department or the student-athlete’s coach prior to the first day of preseason or he or she will not be permitted to participate in preseason practices and games. If your child has participated in a fall sport, he/she will not need another physical as the athletic department already has the form on file.
The Fox Chapel Area Athletic Training Department in conjunction with UPMC John, Chantz, Biedrycki and Associates will provide pre-participation physical exams, at no cost, for high school and middle school athletes’ participation in the winter sports season based on the following schedule (click the link) Physical_Winter2013 .
The online physical registrations are due: Friday, November 8, 2013.
